    Well, I think we can rule the Mg excursion out, since the only places where it goes off the scale are those when the patient was given large Mg injections several hours prior to the blood test.

    I am still reviewing this case; however, I was very puzzled at her Magnesium level. Such an elevation, I would suspect, could cause nocturnal bradycardia, leading to vasovagal response. Also, I haven't yet found any mention of ruling out sleep apnea, which could (possibly) display as lip-smacking, etc., during hypoxia events. ??

    In the data and medical history I do not see a detail of the seizure frequency and the start of the next menstrual cycle.

    The pattern of seizures suggests the seizures may be consistent with the ovulation cycle - but since ovulation alternates between ovaries this would align with the ~50 day cycle and duration would be consistent with the amount of time required in the production of the ovum or in the phase where the ovum is moved into and through the fallopian tube.

    The patients' strong reaction to the TdaP vaccine and the seizure behaviors are indicative of tetanus (mild). This hypothesis suggests there is a localized tetanus bacteria in the region of the ovary/fallopian tube and it becomes an influence during the ovulation cycle. Seizures would not be present during pregnancy as the ovulation cycle would not be active.

    The 'localized' tetanus would not be found by blood tests, ultrasound or MRI. A suggestion would be to treat for tetanus proactively in the period just before the next cycle of seizures to confirm. With confirmation the seizures could be precluded by oophorectomy or will likely cease with menopause.
